Stenzek b2b68c3ced CDImageCCD: Fix LBA count including pregaps twice 
Track start is index 1 but length includes the pregap, so the lead-out
position reported by GetTD was too late.

Stenzek dcb3e3624e CDImage: Support loading CloneCD images 
This replaces the old "img" fallback, which did not work for multi-track
